Enrollment Process In order for your child to be considered enrolled in Evening Star Child Care Services, the parent or guardian must complete all forms and initial all sections within the parent contract.
Evening Star Child Care Services, can provide information to families who are deemed by DCFS standards eligible to receive subsidized child care services based upon family size and total household income. Interested families may contact the Director for requirements.
Enrollment Policy There are several forms that must be completed before we can begin caring for your child. No Exceptions. All forms must be updated at least once a year, or as needed. The forms are as follows:
* Child’s Enrollment Record
* Health History/Annual Physical Examination
* Up to Date Immunization Record (Including Lead screening & TB test)
